Insurance

We do accept some insurance, however coverage for Acupuncture & Bodywork varies widely and will depend on your individual policy. We are currently on several lists as a provider.
We would be happy to contact your insurance company to determine whether or not you have applicable benefits (verification process).
You will be responsible for all deductibles, co-pays, or payments for conditions not covered by your policy.

Medical/Health:
As of January 1, 2014, Acupuncture is now a covered benefit under the Affordable Care Act (ACA), provided your health insurance policy includes Acupuncture benefits.
Section 2706 of the ACA is titled “Nondiscrimination in Health Care”.
If your policy covers Acupuncture delivered by a health care provider (acting within their scope), then your policy must now also cover Acupuncture delivered by a licensed Acupuncturist. (L.Ac.)

If the insurance company is not willing to work with Five Branches Wellness™ directly, you can pay for your treatments and we will provide you with a statement (Superbill) that you can submit for reimbursement.

Auto:
If you have been injured in an auto accident, you probably have accessible benefits (Medpay) that don’t require a referral or participation in a specific network. These programs usually allow you to seek treatment from your provider of choice & we are happy to assist you.

Worker’s Comp:
If you have been injured on the job, you may qualify for Workman’s Compensation. I am an approved provider, & can be referred through your physician.

Health Savings Accounts & Flexible Savings Accounts:

Most people are able to use their HSA & FSA for Acupuncture & Bodywork.  So far everyone has been successful in submitting their claims to HSA and FSA who has used these accounts at our clinic.
